Patient-Centered Medical Home (PCMH)

Regional Family Medicine (RFM) began its PCMH transformation in 2012.  Since that time, we have successfully attained the highest level of recognition for patient-centered care:  Level 3 PCMH Recognition by the National Center for Quality Assurance (NCQA). 


The basic principles of PCMH are:

  • Access to a personal physician who leads the care team within a medical practice
  • A whole-person orientation to providing patient care
  • Integrated and coordinated care
  • Focus on quality and safety
  • Use of advanced technology to assist in decision-making and care coordination

Potential benefits of a PCMH include:

  • Improved access to care
  • Team-based care
  • Higher quality care services
  • Decreased risk of medical errors and duplication of services
  • Less fragmentation of care and improved coordination among multiple providers
  • Decreased overall cost of care
  • Improved clinical outcomes

RFM participates in the statewide Arkansas PCMH Program and was selected for participation at the national level in the Comprehensive Primary Care Plus (CPC+) initiative by the Centers for Medicaid and Medicare Services (CMS).  Additionally, RFM is a member of the Baxter Physician Partners Accountable Care Organization. 

Participation in these programs demonstrate our intention to provide the best care possible while avoiding wasteful overuse of resources.  RFM was recognized as one of only 19 clinics statewide to achieve "Shared Savings" in the Arkansas PCMH Program, an achievement that was marked by demonstrating high quality care while at the same time producing significant savings in per member cost of care for the 2014 performance year.

What does all that mean for our patients?  We make it our goal to provide the right care at the right time with the right provider.  Despite the many moving parts in the healthcare system, the focus has always been to make THE PATIENT central to the care we provide.  We strive to deliver the best quality, value, and patient experience for healthcare in our community.
