If so, we would encourage you to register for our patient portal. It's easy, and will allow you 24/7 access to your health information.
To get started, you will need to provide us with your preferred email address. This can be given to a staff member in-person at our front office, or by emailing your request to: email@example.com. Once you are set up in portal, you can save Portal website to your favorites or you can come back to this website and click the portal button in the upper right hand corner.
